You can print and complete these patient forms in advance, or complete the copy provided by one of our Patient Representatives at check-in. Having these available when you first arrive will allow us to process your information more rapidly and potentially get you treated more quickly.
Board certification through the American Board of Medical Specialties (ABMS) recognizes a physician’s exceptional expertise in a particular specialty and/or subspecialty of medical practice. The American Board of Plastic Surgery (ABPS) is the member board of the ABMS that certifies plastic surgeons.
There are other medical boards that claim to provide board certification to plastic surgeons; however, the requirements, testing and maintenance of credentials may not be as rigorous as the ABMS member boards. Be sure to check the credentials of any surgeon you are considering, and make sure that he or she is board-certified by the American Board of Plastic Surgery.
